To study adverse effects of antiretroviral drugs in the initial three months of therapy in children.Fifty eight patients being newly started on ART & willing to participate in the study were recruited. Patients were followed up for 3 months for clinical and laboratory evidence of adverse effect, follow up on 15th day and further at the end of 1st, 2nd and 3rd month consecutively. High proportion of patients i.e. 44 (75.86%) reported adverse reaction in the initial three months of ART. Gastrointestinal adverse effects were common, seen in 63.63% of patients. Most common in AZT + 3TC + NVP regimen. The initial period of ART is crucial and patient’s perception of adverse effects should be carefully taken into account. Staff training on pharmaco-vigilance could improve monitoring of adverse effects associated with the use of ART drugs in a developing country like India
